求助信格式：

1.求助信多以自我介绍为开头句：

3,求助的内容可以用first, second, last或firstly,secondly,finally或to begin with,besides,what's more, in addition等依次陈述所需哪方面的帮助或者自己遇到的问题。

4.希望对方给予帮助的表达：

写这种信时，应注意以下几点：

1.格式上应采用正式格式，即信头、信内地址、称呼、正文、结束语和签名等。

2.称呼栏中不宜使用具体人的姓名，而要采用诸如Dear Sir/Madam等的表达方式。

3.在对事实情况进行说明时要提供相关的事实依据，同时注意措辞严谨。

我们的小孩名叫曹良映，是个小男孩，年仅1岁，正是在父母的呵护下一天天健康快乐成长的年龄，然而就在今年8月23日，经昭通市第一人民医院诊断，孩子患有先天性心脏病，生命岌岌可危，如果不及时救治，一个年幼的生命即将陨落，留给我们的将是无尽的痛苦。

我叫曹忠益，男，现年37岁，是孩子的父亲，平日靠做点杂工为业，我妻子叫徐西芬，现年36岁，在县城罗炳辉将军纪念馆做一名清洁工，工作。我们夫妇俩是云南省昭通市彝良县角奎镇塘房村老实本分的农民，以前靠种土地过着面朝黄土背朝天的苦日子，在几年前，县城进行城镇建设征用我们土地，我们大力支持，尽管我们家被征去了大部分的土地，生活上更加没有渠道，但我们懂得先有“大家”才有“小家”的道理，支持国家搞建设我们毫无怨言。我的大儿子现读小学四年级。两个孩子活波可爱，这让我们有了许多的安慰，我们夫妇俩每天起早贪黑，脚踏实地，做许多人不屑的苦活脏活，靠微薄的收入艰难地撑起一个家。我们每天艰辛的生活就是吃了没文化的亏，因此心中只有一个信念，靠自己的劳动挣钱养家糊口，再苦再累苦也要把两个孩子抚养长大，让他们好好读书，好好做人，好好成长，成为社会有用之人。

彝良是云南东北部的一个小县，是国家重点扶持县，是红军长征时曾经过和战斗过的一块革命老区，去年9月7日11时19分和12时6分，彝良县遭受双震袭击，9月11日再次发生暴雨灾害，多灾叠加，是彝良有史以来破坏性最强、波及范围最广、救援难度最大的自然灾害，直接经济损失达34亿元，因灾遇难80人。地震期间，我们亲眼见到了和感受到了心存大爱的无数社会各界好心人为了彝良灾区人民从废墟上挺立脊梁，恢复重建家园的所作出的努力，这让我们感激万分。我的小儿子正是在地震期间降临人世，他的出生给我们家庭带来了更多的欢乐，我们真正感悟到了生命的可贵，在现在和今后，我们会教育孩子们懂得感恩，传递爱心。

在得知小儿子的病情后我们一筹莫展，抱着被病痛折磨的孩子，我们无可奈何，每天都以泪洗面，也许孩子生错了地方，他出生在的是一个家境较好地家庭对他的生活就会更好。医生告诉我们，如果孩子能及时治疗的话或许能挽救生命，但至少需要几十万元的治疗费用，这笔费用对于我们这样仅能靠体力劳动维持温饱的贫困家庭来说是一个很大的天文数字，我们根本无能为力，而我的亲戚、朋友他们能够给予的也是少许救助，只能是杯水车薪，无济于事。我们特写了这封求助信，希望能得到社会上慈善组织和医疗单位的救助，挽救孩子的生命，让我的孩子在无数个好心人的帮助下，年幼的生命之火能够重新点燃，我们将感激不尽，我们更会胸怀感恩之心，在各自岗位上加倍努力工作，以实际行动报答好心人的关爱!
